Missed Appointments (No-Show Policy)

We understand that unexpected events can happen. However, missed appointments without prior notice can impact the availability of care for others.

No-Show Fee:

• A $45 fee will be applied if an appointment is missed without prior notice.
• This fee helps us maintain availability for other patients and ensures our commitment to timely and efficient care.

We kindly request that you contact us at (773) 862-3180 as soon as possible to reschedule or cancel your appointment to avoid this charge.

Late Arrival Policy

Timeliness is essential to provide all patients with the full benefit of their scheduled appointments.

Late Arrival Fee:

• A $20 fee will be charged if you arrive more than one hour late for your appointment.
• If you anticipate being late, please notify us immediately at (773) 862-3180. We will do our best to accommodate you or reschedule your appointment.
